I'm often amazed at what people have done to get something to work. While it's good that it worked there is often a simpler way of doing things. Which I know all too well with having gone around the long way myself many times. A couple of things that have helped me to do an OS install when running into this problem. First update the driver for the Hard drive. This is I think the easy fix for many people with these Install problems, Updating to OS 8.1 from an older OS maybe the first time this showed up . The driver for the hard drive is simply too old to allow an install to happen. This is true for OS 8.1 and perhaps OS 9.1 .You do not need to boot from the CD pop it in and find drive setup in the Utilities folder. If not using the Apple driver make sure the version you are using will work with the OS you are installing.Just do a driver update unless you want to wipe the drive. Many people don't realize you can do an OS install without booting from the CD. This has been possible since maybe OS 8.1 It is a good thing since some clone machines can't boot a CD with the original CD player. I'm not saying you shouldn't try to boot from the CD to do an OS install. I'm saying you have options if that doesn't work. The other tip if your not booting from the Cd and if Booting from the CD didn't work . There are two things to choose from for doing the install. The one most people will choice I would think is called " Install Mac OS 9.1" well if that doesn't work try the "Mac OS install" worked for me after having problems with the other one.
