When I have this problem with that card, the only way I have been 
able to fix it is to connect a Mac to VGA adapter to the Mac port 
(DB15) on teh video card and connect the monitor through that.  Then 
set the adapter dip switches to something like 17" or 20" 
Multi-synch.  Of course, that means you need an adjustable adapter 
with settings for multi-resolutions.

There may be other better solutions, but this is the best I've been 
able to manage.
