I think you're on the right track.

If the page doesn't present a form to be filled, no form manager or password manager will respond.

I suspect IE on the OP's machine is set to accept all cookies and save them forever, so when the OP returns the cookie is still there and the site reads it, so it doesn't ask the question. SM is set differently (there are dozens of possible settings) so the cookie is not already present at initial page loading and the site asks the question.

Just for an example, I have my machine set to accept /only/ session cookies, which means it clears all cookies every time I shut SM down. In my case, the site would always ask the security question.

In Internet Exploiter 8, you can now set the program to clear all cookies on exit if that is your preference:

Tools | Options | General tab

(You can get to the same dialog without launching the program via Control Panel | Internet Options)

Under "Browsing history," check the box "Delete browsing history on exit," and then click the "Delete..." button (don't worry, this won't actually delete anything yet). In the "Delete browsing history" dialog that appears, check the box for "Cookies" and any other elements you want to delete at each program shutdown.

Caution: These deletions may not be implemented in case of a program or system crash.

IE does have a rudimentary form filler, called AutoComplete. For more information, search that term in their help file.
