I installed it from the place you gave and was excited for retrieving
the traditional classic theme of netscape/mozilla/seamonkey 1.x
unfortunately some icon are not in place.
forward keeps the one of default SM 2.X
and reload gets no icon at all

is it some way to get all in place ?
No problems here.
Forward (email) has the classic 1.x icon.
Reload, I have never seen a reload icon in the email section
Reload in the browser part has the old classic 1.x icon
Just like the one in the screenshot at the add-on site

You can try to add the reload icon to the bar.
Right-click the navigation bar, choose customize, drag & drop the reload
icon to the navigation bar.

got it !
I was using small icons, still a problem in this case...
and if you show help content (F1), no icons at all..
also something to do around home icon in user's bar
