I posted some about this a few weeks ago but never received a response.
This does not happen consistently! When I right click on an item in my personal tool bar or a URL in Live Journal (for example), and then select Open in New Tab, I quite often get two new tabs.
The first new tab contains a name for the function like Google, or Anything and Everything which is associated with my Live Journal. The second new tab contains a url (sometimes modified slightly). If I click on Reload the second new tab opens the desired program.
At times, I also get a very similar result in that sometimes the first new tab does not appear, just the second one. And that need a reload to open it.
