Extensions - Yes, there should be a "Remove" button to the right of the "Enable/Disable" button.
Appearance - Same as above.
Plugins - There is no "Remove" button next to the "Ask to activate/Always activate/Never activate" button. They have to be removed at the operating system level, usually by uninstalling the plugin or program that installed the plugin.
