The name column is not in the drop-down menu because it is supposed to
be shown always. Is it possible that the left edge of the second column
got moved to the far left, thus hiding the name column?

This can happen
mysteriously -- without any action by you -- from a momentary fault of
your computer. Such a fault could be serious, indicating a pending hard
drive, memory, or processor failure. Or it could the the result of a
power fluctuation.
