you have to set up port mapping for SIP (udp5060) and RTP (udp upper ports).
I guess you only have a single public IP, so you'll need to be creative.
if you can, get the sip clients to bind both sip and RTP to a specific
ports (different in each case) so that you can nat inbound and outbound
traffic with 100% confidence.
But, yes, sip proxy is easiest. can you not run it on a machine inside
your network? then you only need to sort out one set of natting!
