Is the CPU capable of keeping up the OpenVPN encrypting? Perhaps you need more CPU or RAM for your firewall(s). Another thing to try is a better codec. I personally use G.729 on all non-local SIP clients. It works extremely well on slow long-haul links, and the G.729 codec license for Asterisk is pretty cheap from Digium.
