Surprisingly this turned out to be due to traffic shaping; I say
surprisingly because all I remember asking for was prioritisation of
VoIP and nothing else. However, just switching shaping off didn't seem
to help (although other advice I received suggested that rebooting the
pfSense box may have made a difference here, I didn't test that).

Eventually I configured shaping with the wizard to set both LAN and OPT1
parent queues to 100Mb/s explicitly, and I got the performance I had
been expecting.
