Why not use Super Stereo mode?  After all, it
was intended for "decoding" stereo recordings.
Geoffrey Barton posted the latest (1985)
decoding equations for this.

This information used to be in the
Ambisonia.com Wiki.  Until the Wiki is
resurrected somewhere, and rather than
repeat Geoffrey's post here, I have made it
available on my Google Site at:

Look under Ambisonic stuff.  As Geoffrey
points out, Super Stereo uses the same shelf
filters as B-Format, so that should make things
easier for you.
