Unfortunately I've only ever heard two, and neither was what I'd call a permanent setup. The first was kindly set up by Eero Aro, ad hoc, and the second was demonstrated to me by Ville Pulkki in an anechoic chamber of what was then called Helsinki Polytechnic (now Aalto University).
As I said years ago, I'm a theorist, not a practitioner. I'd really like to have my own rig, but unfortunately I've never had the means to build one. Even for basic pantophony -- there the first investment would be a room where it works even semi-right, and I can't afford even that. :)
