Perhaps I do not understand what it is you are after. It seems that you are ignoring the question of "how to get from B-format to 5.1", which is a separate, matter that has nothing to do with a test wavefile, as far as I can see.
If you want to place the 5 speakers "accurately" without using a tape measure and protractor, one could use test recordings made in 5.1. The three front channels and the two rear channels are easily dealt with. The slight problem, solved by a little trigonometry and a calculator, is to get the ratio of sound split between LF and LR accurate to achieve a phantom source at 90 degrees. I made a test DVD-A that achieves this for 4.0, which is, of course, easier. Then we are actually talking about the use of the B-format channels with very little manipulation.
