I don't have docs for this modem, but this looks like a fairly simple
layout of pins.  Jumpers will only go horizontally, connecting pairs
of pins -- pair #1 is at the bottom, #6 at the top, etc.  The top four
pairs will select the IRQ:  pair #6 for IRQ 2, pair #5 for IRQ 5, and
so on.  You want IRQ 3, so jumper pair #4.  Leave #3, #5, and #6 open.

The bottom two pairs of pins will select the port address.  Probably:
pairs #1 and #2 both open = COM1, pairs #1 and #2 both closed = COM4.
COM2 will have only one pair closed -- #1 or #2, but not both.  Pair
#1 closed seems more likely, at a WAG.
