This looks pretty much like a scsi bis error. I suggest you check these items
(in this order):

- tape set to a known unused scsi target i.d. (usually 4,5 or 6)

- proper termination of the external scsi bus. In particular, if you are using
  wide scsi disks along with a norrow scsi tape drive, the following
conditions
  must meet:
        - wide devices located near to the host adapter.
        - wide/narrow converter must have built in terminator for the upper 8 bits.
        - wide scsi cable should not exceed 0.5 meters
        - narrow cable between wide devices an tape should not exceed 0.5 meters.
  please note: the restrictive cable length requirements are due to the
               mixed mode (narror/small).
