xen: create a Xen nexus to use in PV/PVH
Introduce a Xen specific nexus that is going to be used by Xen PV/PVH
guests.

x86/xen/xen_nexus.c:
- Introduce a Nexus to use on Xen PV(H) guests, this prevents PV(H)
guests from using the legacy Nexus.
