pluto: fix IS_IPSEC_SA_ESTABLISHED as per Antony's findings
Cleaned up and re-ordered the macros a bit for clarity.
IS_V2_ESTABLISHED should be phased out once we label our children
correctly as child sa instead of parent sa.
