pluto: eliminate STATE_IKE_FLOOR, increase STATE_IKE_ROOF
The state STATE_IKEv2_ROOF lurks in the code so reserve space
for it in tables.
When iterating over states, deal with IKEv1 and IKEv2 separately -
don't assume they are somehow contigious; and don't expect
STATE_IKEv1_ROOF in working code.
