ikev2: re-implement stf_status v2_record_outbound_fragment() by mimicking
open_v2SK_payload()
The SK and SKF headers are different, and the calling code has
different information available (noteably, the fragmentation code does
not have/need MD). close_v2SK_payload() can be used.
Delete no longer used {start,end}_encrypted_payload(),
ikev2_padup_pre_encrypt(), ikev2_authloc(), and
ikev2_encrypted_portion.
