state: replace FOR_EACH_COOKIED_STATE() with FOR_EACH_STATE_NEW2OLD()
As suggested by 1b9bb50778982fd584baa13dc08fd0f85c4e78e5.
Helps clarify that functions like flush_pending_children() iterate
through all states (and not, perhaps some subset based on cookies)
and why it could find an IKE SA re-key state hashed to the wrong
slot.
