ikev2: clarify code hacking MD when processing a COOKIE response
The problem is in complete_v2_state_transition() and not
ikev2_parent_outI1_continue() (in fact the latter doesn't even use
MD), and the reason the MD is being hacked is so that it looks like it
doesn't exist.
