ip: cleanup rangetosubnet(from,to,dst)
Use address_is_unset(). Eliminate pointer indexes. Replace call to
initsubnet() with (new) simpler subnet_from_address_maskbits() (for
this code path all the checks in initsubnet() were redundant). Delete
a LOGGER=NULL hack.
