ikev2: use a state transition when initiating a CREATE_CHILD_SA exchange
Fill in missing transitions for rekeying IKE and Child SAs, and
creating a new Child SA. While the transitions are separate
the .processor functions are still implemented using common
code (sigh).
Pass both the larval state and the state's first transition
into v2_msgid_queue_initiator().
Make function names more consistent.
