ikev2: overhaul ikev2_find_host_connection()
used by IKE_SA_INIT responder
- replace find_host_pair() with FOR_EACH_HOST_PAIR_CONNECTION()
- drop IDi check, not applicable to IKE_SA_INIT
- drop policy excludes, always LEMPTY
- explicitly skip ID_NULL+INSTANCE
follow up #584 and discussion with DHR
