connections: only update .instance_serial when an instance
Add .next_instance_serial and use that to track serial numbers.
An established security label instantiates the connection twice:
template -> parent -> child
but only the child has a non-zero .instance_serial.
(instantiated security label connection names could do with
a re-think, perhaps incorporate the label, or perhaps
include multi-level serial numbers)
