documentation: use ENTITY includes to build ipsec.conf.5.xml
- add top-level ipsec.conf.5.xml file
- use XML ENTITYs to include sub .xml files
- make certain sub .xml files are well formed
- run $(TRANSFORM_VARIABLES) on output; not input
- replaces "cat $(XMLINCLUDES)"
The alternative here is to use xi:includes but they proved
more tedious.
