ikev2: add/use STATE_V2_IKE_SA_INIT_IR (for want of a better name)
On completion of the IKE_SA_INIT exchange, the IKE SA initiator
transtions into IKE_SA_INIT_IR. From there it can start either
an IKE_INTERMEDIATE, or IKE_AUTH exchange.
