packets: return bool from close_pbs_out() and open_*_pbs_out()
(yes currently always returns true)
for consistency with other calls which all return bool

packets: set the small packet size to approx max before fragmentation
and call it fragment_pbs_out; code filling this buffer
needs to implement fragmentation (which is true of IKEv2)
