connections: tighten visitor's requirement that caller holds a reference
Always take a connection reference before calling
the visitor; which means on visitor entry:
connection's refcnt > 1
and on visitor return:
connection's refcnt >= 1

connections: always hold reference when visiting connections
this means on entry to a connection visitor there's at least
two references, an on exit, at least one (because the visitor
may delref).
