I have
this game for PC, and I only have the case and manual. I'm not sure if
this game was ever released in a box.
That
being said, I doubt its value for CDTV is very high.

This game have any value CD case and CD manual only for
CDTV? Didnt' even know what CDTV was til I looked it up, Commodore computer,
first computer with CD drive?
