You just need to add your wmode value to the !IE code block. When using static publishing, you have to define all flashvars, params, and attributes twice. For this reason, I recommend people use the SWFObject code geneator (link in the wiki) as it will ensure your code is correct.
Once that is in place, you can either use the inline order of elements to determine you z-indexing, or specify then explicitly.
