I cannot reproduce (at least on a mac). The banner with the blue shoe always
appeared for me on the 5 times I tried.

Given that your main swf is displaying, this is as far as swfobject gets
involved. If there are weirdnesses within your swf, then this is s purely
flash related bug.

I have found in the past that most of these issues are around how the
browser is caching assets etc. You can always try tacking on a unique
querysting like  "banner.swf?id=" + new Date().getTime();  (this will force
a fresh download on each view). See if that gets rid of your issue - if so,
you  know what direction to look in :)
