http://www.sipwise.com and quite impressed. Also the Web Admin GUI is nice and functional. There is "community edition" for free, downloadable as package or preinstalled VM (Debian). If you're environment is growing and you need high availability, professional support or SLA, you can upgrade to "Provider" or "Carrier Edition" and handle up to 200k of concurrent lines. Just have a look at their Web Page and give a try on the VM.
