Looks like a module issue to me, and is probably caused by lack of attention to attribute stacking.
USFM does not support stacked attributes, so the problem may be something in the conversion to OSIS. Methinks we should add some advice in the developers' wiki about the problems that are likely to occur with attributes not being stacked.
