I would like to jump to step D8 and Congratulate the Team. We may have skipped a few of the earlier steps, but we have a solution that works. Step D7 is "Prevent recurrence: Modify the management systems, operation systems, practices and procedures to prevent recurrence of this and all similar problems." So before we put this thread to sleep, please all have a good think about what we might put in place to follow this sound advice.
