I don't know how he edited it, but the import file has plain spaces, and they work fine, e.g.: <item><reference osisRef="Shaw:Shaw/The Reformed Faith/Preface">Preface</item> <item><reference osisRef="Shaw:Shaw/The Reformed Faith/Essay">Introductory Essay</item> Interestingly, most of the closing </reference> tags were missing, but it still worked.
