I am still waiting for a fix to... I think we are between 5 and 10 to have 
reported this issue witch
is quiet critical... The quick fix i have done is cron that clear the cache 
every night and another that
call every module of my backend app to re-generate it...

In my case : no accel / sf 0.6.3 / only happens on admin genrerator modules

Has anyone gotten to the point of being able to reliably reproduce  
this issue, or perhaps see it occur with any sort of regularity? For  
those experiencing this issue, can you all indicate whether or not  
you're using one of the available PHP accelerator / optimizers /  
opcode cachers? I expect most people are, and it would be interesting  
and pertinent to see if this problem went away if the optimizer was
