I found the problem:
prior to 1.2.0 path_info_key was set in settings.yml, but for 1.2.0 it
is set in factories.yml, such as:
request:
class: sfWebRequest
param:
path_info_array: SERVER
path_info_key: HTTP_X_REWRITE_URL
(also added this info to the ticket)
