Look into segment separators options for sfRouting:
http://www.symfony-project.org/reference/1_4/en/05-Factories#chapter_05_sub_segment_separators
This can be enabled for all routes or for specific ones.
However I'm not sure this will work for you:
/myroute/my-item-56
maybe with
/myroute/my_item-56
