I learned the hard way too. Paying close attention to the repositories on Github is a good way to keep up to date on stuff. You don't need to read that stuff constantly, but absolutely take a look at the update files, the latest commits, and pull requests before upgrading anything. If in doubt, create a new branch of your project, upgrade, fix any issues, and then merge the changes with your master branch. This isn't so fun if you're using Subversion but it's pretty straightforward with Git.
