It's really easy. You go to the branch in GitHub (in this case, to http://github.com/smichr/sympy/tree/2003). Then, you click on the button that says, "Pull Request," to the right of "smichr/sympy". Then, it will take you to a form where you can fill out a comment to append to the pull request. By default, it will send it to Ondrej, because you originally forked from his repository. It will say something like "You're asking *certik* to pull n commits into 'certik:master' from 'smichr:2003'". You should click on the black 'certik:master', and change it to 'sympy/sympy:master'. Then click on "Update Commit Range". You should be good to go at this point, and you can click on "Send Pull Request" to send it out.
