Thanks for the links - and I will use that for one problem posted (adding info 
to the _link roads from toll plazas) - but the bigger question was about the 
motorway_junction being placed on exit nodes, and how that applies to entrance 
only junctions.  It seems motorway_junction is only for exit ramps, if I am 
reading it correctly. 

If we are using junction to just define the exit point on the motorway, There 
should be some tag for the entrance and exit nodes on the normal roads to 
define, in some complex situations, what entrance is named what without using 
just the ref tag, I think - similar to how a subway has entrance tags with 
their own ref and a separate station name with its own ref. Many junctions 
wouldn't need it, but complex junctions (and the motorways in Tokyo are 
insanely complex) might require additional information for helping a driver 
both visually and spatially match the map/navigation instructions to what they 
see in the real world, beyond a lane and when to turn, because in some places 
it is a confusing spaghetti mess and 50m makes a huge difference.
