I would use both lanes=x+1 (where x is the number of lanes without
turnout section)
and something like

 turnout:lanes=no|no|yes|yes (if there are 2 turn out lanes)
and
lanes:turnout=1 (or 2, or 3)

this is similar to tagging deficated bus lanes
