This is actually not a contrived case: happened to me on a certain
conversion.
It happens as follows: dir/ with a1, a2, a3 gets added. Then gets removed.
Then dir/, now containing z1, z2, z3, gets added again.
Some targets end up with six files in dir/, instead of three.
I'll be sending a test case to the list shortly.
