Hi, As one of the people who have worked extensively with Swedish road numbers on OSM, I might be able to give some answers. I should emphasize that I have no legal or engineering training. The following is based on discussions within the Swedish OSM community, my own reading and discussions with my father, a road engineer with long experience of dealing with road management at the local level. The discussions are spread out over the mailing list, the wiki and to some extent IRC and IRL meetups. I tried to collect them at one point, but I don't think I found all of them.
You got the road hierarchy basically right. There are some more details, but I don't think they are relevant for this discussion. The legal instruments that define and list Swedish state road numbers are linked from [1]. It has been our interpretation that the lists count as laws not covered by copyright [2]. Therefore, we have gone through the lists (the texts, not the accompanying maps, which are copyrighted) to check that numbers are correct, to standardize ref=, to add numbers and to discover missing roads, which, if possible, have been added from regular OSM sources. This work is not yet complete in all parts of the country. There have been some claims that county registration letters (länsbokstäver) are no longer part of road numbers, but this seems to be contradicted by the legal instruments, which use letters for roads in other counties than the one covered by the list in question. In OSM terms, the number without letter would be a reg_ref=, and the number with letter would be a nat_ref=. I'm personally agnostic as to which of these should be put in ref=, but I follow the consensus as I understand it, which is to use the letters in ref= in order to decrease confusion around borders (a somewhat extreme example [3]). Joakim Fors [4] has written some code that generates relations from ref=. It has been run in Skåne County and maybe others.
