If using an extract, then yes above looks right.
There is also a guide here:

The re-import and restart steps are dropped if you are using a
minutely setup, but I am not sure how minutely and extracts work
together:

Have a look at TileMill for creating your own styles, they have a few
base styles worth looking at, alternatively there is a new OSM base
style under development here which is a good starting point:

TillMill is VERY impressive. It can be used server side too AFAIK.
