Mine, from April via a SalesForce instance, don't have that:
Content-Type: application/pdf; name="Dose_Admin_....pdf"
Content-Transfer-Encoding: base64
Content-Disposition: attachment; filename="Dose_Admin....pdf"
Very strange.
