Not sure if you had another response on this.  I'm frankly impressed at how far 
you got on this form.  Maybe the issue is that I'm only on WE 8.4 here in my 
office and maybe this site would respond better with WE 9.  What I see visually 
where you pick a date is a calendar view for March and April --  the grid 
format with the days of the week across the top.  I found that if I did a 
Insert Tab to pull up the link on the page, the calendar is actually labelled 
really nicely "you are on Friday, March 13".  If I hit on that link in the 
dialog I'm placed on the calendar within the page and can tab between the date 
links.   I was only finding the pickup location combo box erratically, so I 
don't' fully trust that my experience is like yours, but it's worth a try.  I 
was using Firefox.

tried it with NVDA.

Now here is where I have a seemingly insurmountable problem.  You are 
presented with a combo box for selecting whether the package is to be 
picked up on your porch or in your mailbox when the carrier delivers 
your mail.  This is okay, but the date selection mechanism just doesn't 
work for me regardless of screen reader though there well may be 
something which I am missing.  So the question becomes "is it possible 
to complete the scheduling of a package pickup using Window-eyes 9.1?

Thanks in advance for any assistance.  By the way, I had similar results 
regardless of whether I used Internet 11 or a recent version of Firefox.
