After you press alt-control-q the diagnostics starts. If you have the user account control enabled, you'll need to press alt-y to allow the diagnostic to do its thing.
It will say progressing progressing for possibly a couple minutes. Finally a dialog comes up where you enter your contact details as well as what went wrong. Tab through the dialog and fill out the information. Finally hit the send button. That should do it.
