Ethics likely have nothing to do with it.  Just because something is open 
source doesn't mean it is free to anyone who wants to use it.  Usually open 
source 
software will be released under a particular license.  Some are completely free 
to anyone while some are free only for noncommercial use.  NVDA is going to 
be able to use some software that AI Squared can't use because NVDA is itself 
open source.  It is very possible that E-Speak is open to anyone, or AI 
Squared might be paying something for the right to include it, I do not know, 
but I have some confidence that they are doing what they need to do to 
include it.  

While Android is an open source operating system, that does not mean anything 
running on it is open source.  The developers of Android developed it 
specifically, as I understand it, to have an open source platform for 
development of commercial and noncommercial products.
